In January 1998, Trinity Institution and Homer Perkins Center were legally merged to form the current corporation to establish a stronger integrated multi-service agency. Today, Trinity Institution-Homer Perkins Center, Inc. continues to be a private, not-for-profit, non-sectarian, social service agency devoted to meeting many of the social, economic and educational needs of the community.
On January 11, 2006, more than 100 people attended the ribbon cutting ceremony for the Family and Neighborhood Resource Center. The Resource Center has multiple entry points to meet the comprehensive needs of families including an array of health and wellness activities and other services listed below. Children and families who participate in any one of our programs will have access to the complete array of services we offer.
CARE:
The After School CARE (Crafts, Arts, Recreation and Education) program offers tutoring assistance for homework, mentoring, reading club, chess club, character education activities, violence and gang prevention, drama lessons, music/dance classes, robotics instruction, science club, bicycle repair and more. Evening Enrichment:
Evening Enrichment serves children 15-18 years old and provides recreational, educational, and cultural programs. The program runs Monday through Friday from 6:00 p.m. to 9:00 p.m. Contact Greg Foskey, ext. 109 for more information.

Bicycle Repair Workshop:

Early Intervention and Family Support Services :
Early Intervention and Family Support Services provides homebased services to families who have a child birth to five years who has been identified as having a developmental delay or who is considered to be "at risk" for developmental delays. This program is designed to assist parents in providing a loving and nurturing environment for the children while simultaneously enhancing their child's development through play. Areas of concentration include: cognitive, language, fine and gross motor and social development. A parenting and children's group is offered to participating families. Family Reimbursement:
Family Reimbursement provides assistance to families who have a family member living at home who has a developmental disability. This program is designed to assist families in meeting specific needs they may have in caring for their developmentally disabled family member living at home (i.e. respite, adaptive equipment). Success By Six:
Success By Six: like Early Intervention, this program serves pre-school children and their parents/guardians who require intensive engagement, case management, advocacy, and support in order to ensure successful school experiences and family interaction. This program is a collaboration of Trinity Institution-Homer Perkins Center and the United Way of the Greater Capital Region. Emergency Food Pantry:
The Emergency Food Pantry provides food to individuals and families who receive unemployment, social security, disability, food stamps, or who are victims of fire or similar catastrophes. Beneficiaries of the program are referred by local agencies, churches or other area institutions, or come in on their own and are provided with emergency food. Applicants must have need based on income, provide three forms of identification at initial intake, and live within the agency service boundaries. Hours are Monday through Friday from 10:00 a.m. to 5:00 p.m. with the exception of 12:00 noon to 1:00 p.m. for lunch. St. Patrick's Home Based Partnership Program:
St. Patrick's Home Based program offers small business opportunity assistance for people who would like to operate a registered home care child provider. The program focuses on training to meet state registration application requirements, one on one technical assistance, support group for providers, and state registration application. Parenting Classes:
Parenting classes for parents young and old. Topics include, rules, roles, and responsibilities, child and family development, age related discipline and communication techniques, keeping our families safe, anger and stress management, routines and rituals. Trinity Institution offers an array of support services and information about jail, prison and parole. Learn how to help someone survive the prison experience. Our agency offers individual and group instruction to repair personal credit, learn about banking to avoid predatory practices, start entrepreneurial ventures in order to progress financially and build personal wealth. Homer Perkins Center:
Homer Perkins Center offers a 6-12 month comprehensive individualized program that addresses the varied needs of the addicted person. The center promotes a drug free lifestyle modeled on the 12 step self-help program that incorporates the educational, social, spiritual, emotional, mental and choice making principles of recovery. Transitional Housing:
Residents who have successfully completed the Homer Perkins Center comprehensive substance abuse treatment program are eligible to transition to two apartment buildings owned by the agency. These apartment buildings are located close to our service area. While residing in the transitional apartment, the resident has access to supportive case management services including oversight of the resident's sobriety support plan, employment search and job retention support, and assistance with all housing and income maintenance needs. Prevention Program:
Trinity's Prevention Program provides intensive home based services to families with children birth through sixteen years of age who are at risk of foster care placement or are in placement. Services include assessment, individual, family, couple and weekly group sessions. Advocacy and coordination with families, school, and other service delivery systems are also provided. Detention Diversion Program:
The Detention Diversion program is an intensive, home and community based program focusing on youth 14 through 18 years of age and their families, who are at imminent risk of out-of-home foster care placement. Participants receive frequent contact with their family worker who focuses on advocacy, case management, trauma and conflict resolution, and behavioral change so that the youth may remain at home. The worker also works closely with the family. Youth who take part in the evening enrichment program learn a variety of skills. As with all program participants, the youth in this program and their families are eligible to participate in the range of health and wellness programs available at the agency. Jail Transition Program:
Jail Transition Program is a four-week cycle designed to help incarcerated youth, 16-20 years of age, turn their lives around using a 12 Step Program model. The goal of the program is to remove obstacles to training and employment for young people involved with the criminal justice system. We provide community re-entry services as well. Target Program:
Target Program is an intensive targeted to youth who have demonstrated an extreme high risk of involvement in criminal activity. Services include case management, individual and family needs assessments with referrals and anger management and peer recovery group facilitation. Arrest Diversion Program:
Arrest Diversion Program provides an alternative to arrest for juveniles 10-15 identified by the Albany Police Department and Albany County Probation as eligible. 192.1 Mile Walking Club:
The 192.1 Mile Walk Fitness Club is open to the South End community. The program runs from Monday through Friday from 8:00 a.m. - 9:00 a.m. When a participant reaches 192.1 miles he/she will receive a "192.1 Mile Walk Fitness Club" t-shirt. Blood Pressure Screening:
In keeping with our theme of healthy living, Trinity offers free blood pressure screenings for the public every other Friday. In addition the agency sponsors other health care screenings including the diagnosis of diabetes, kidney disease, lung disease and more.

Dancing with our Elders:

Youth Empowerment and Mentoring Program (YEMP):
With HIV prevention as the primary focus, this curriculum-based program provides youth with the supportive, nurturing environment that allows them to take leadership roles in the community. Utilizing the benefits of peer education, our youth are empowered by program staff to provide outreach services to local schools and their neighborhoods throughout the year. Utilizing video technology, computers, theatrics, and educational campaigns, the program is able to reach a vast number of youth with a message designated to reduce risk taking behaviors of any kind, but especially promote safety regarding the transmission of HIV/AIDS. After School/Summer Day Camp Programs:
Afterschool Crafts Arts Recreation & Education (CARE) Program provides a structured, supervised, nurturing program for children 5-12 years old, and involves parents in the administration of the program. T-LEAP:
Trinity Life Employment & Advancement Program assists individuals with a criminal record to find and retain employment and retain positive, healthy relationships with their offspring. Our T-LEAP program only accepts referrals from the Albany County Probation Department.
